A rate hike seems reasonable. A 56% rate hike does not. Four and half dollars per month will pay for a lot of trips between your house and the next.

When was the last time a community needs assessment was conducted by an independent group and used to make hard decisions? Is there a current resouce inventory? To make business decisions you need input tempered by data from every community group affected (not only physicians), but the needs of the community must take priority. Available resources need to take care of critical community needs first and as resources dwindle... services must also.
Planning for community needs and implementing those plans is critical to success.

How widespread is the incompetence that allowed this situation?
Should those responsible for oversight of the hospital be removed?
Why were far-sighted steps not taken to keep the hospital solvent?
How can the public trust that their resources are being use appropriately?

Is this a case of management by crisis? Does NRMC track activity of staff and services to determine productivity and user levels? Why don't they understand how to control expenses? It appears they do not know where the problem areas are or how to correct them.
Strong leadership is needed to institute the controls and make the tough decisions necessary. Who will that be?

More detailed and comprehensive reporting on this issue is required.
Why are they only collecting 46% of billing? That would bankrupt any business.
When competition increases you reduce your expenses to match your expected loss of market share. You can look for ways to broaden your market or increase your competitiveness which might increase your expenses.
You institute activity measures for staff and other expenses to determine what is required. Then you act to conserve resources.
When this car wreck happened who was asleep at the wheel?
We need a strong leader who accepts responsibility and makes timely decisions. Who will that be?

On Fortenbery must identify donors
